Things are a bit different this year with the impact of Covid-19.I have attached both the Diet Modification and Food Allergy and Anaphylaxis forms. We ask that these forms be updated yearly for our school records. These forms can also be found on the Pekin website. Please have the forms completed by your primary care physician and returned to the school. You can bring them to open house/registration/mailed or faxed to the school.Please remember if your child requires medication or an antihistaminesuch as Benadryl or an Epi Pen related to their allergy or intolerance you are asked to bring that item to school in its original packaging as the school does not supply those medications.
