Neurosurgery for Spine and Brain Disorders
Despite advances in the art of microneurosurgery, the risk of surgeryfor brain and spinal diseases is still significant. It is even greaterfor surgery involving skull base tumors or vascular lesions and complexspinal diseases. Escalating health care costs are another important concern.Ideal neurosurgical treatment should provide patients with cure of thedisease, minimal risk, and rapid recovery as well as return to employment.
Dr. Jho has developed various innovative minimally invasive surgical techniques for spine and brain disorders that have now been applied to patient care. Among thesetechniques are endoscopic transsphenoidal pituitarysurgery through a nostril without skin incision or nasal packing, endoscopicskull base surgery through a nostril, a nose-bridge incision craniotomyfor skull base tumors, an orbital roof craniotomyor superolateral orbital craniotomy through a small eyebrow incision fortumors and aneurysms, a subtemporal approach through a small temple incisionfor skull base tumors and aneurysms, and a retromastoid approachfor tumors, cranial nerve disease, and aneurysms. Postoperatively, patientsrequire only small incisional bandages. Delicate microsurgical techniqueswhich utilize an operating microscope and an endoscope help to achievesuccessful treatment of brain lesions through small and precise exposure.Brain retractors are never used in order to avoid unwanted brain damage.Many of these procedures can be done on an outpatient or short hospitalstay basis.
New surgical techniques for spinal disease have been developed aswell. These innovative procedures include a disc-perserving cervicaldisc operation, microdecompression of the spinal cord for cervicalstenosis, anterolateral or posterolateral approach for cervical spinalcord tumors, outpatient thoracic discectomy, outpatientlumbar microdiscectomy, microdecompression for lumbarstenosis, etc. Those operations are performed mostly on an outpatientbasis. If required, spinal instrumentation and bone fusion are also performedby minimally invasive techniques.
