If in doubt or have questions, contact Client Services well in advance of camp.Students Also May NEVER:view pornography or any inappropriate material (including sexual content, inappropriate violence, racism, bullying, etc.) on the Internet; steal property of others (If a student is suspected of possession of property which is not their own, the student may be subject to search and seizure); inappropriately use equipment; engage in internet hacking; use or possess weapons (If possession of weapons is suspected, student will be subject to search and seizure. If a student is found in possession of a weapon student will be immediately dismissed and permanently banned from all iD Tech facilities; bring pets or animals to any iD Tech location; (Service dogs may be permitted.) bring drones, hover boards, roller blades or shoes with wheels; sign up on third-party websites without the permission of their instructor; use false information to sign up on third-party websites; share personal information with staff members or ask staff members for their personal information; contact staff members on social media or in any other form outside of camp; and leave camp without checking out with an instructor or other appropriate camp personnel wear see-through clothing or exposed undergarments or accessories or clothing that have statements or pictures that are offensive in nature, including but not limited to, statements or pictures referring to violence, drugs, alcohol, gang affiliation, sexual suggestions or racial slurs.Students and Parents/Guardians MUST:be responsible for all keys issued to them; not use personal electronic devices during instructional time - unless they are approved by the instructor (It is recommended that personal devices be locked in a dorm room. Health and SafetyI understand that physical and recreational activities contain inherent risk of injuries. I certify that my student is in good health and able to participate in iD Tech programs, and that if for any reason my child should be restricted from participating in any activity, that I have provided iD Tech notification of such restrictions through the health forms associated with my student’s account. I authorize staff to attend to any health issues or injuries my student may incur while participating in an iD Tech program. This includes providing basic first aid, dispensing authorized medications, and seeking emergency medical care for my student should they deem it necessary. I agree to be financially responsible for any medical treatment needed during the duration of the program. I understand that I am responsible for providing the sole insurance coverage for any medical treatment my student receives, and agree to release any and all records necessary for my child to receive medical treatment. I understand that during or after completing the registration process, I will receive a confirmation that includes the information necessary to access My Account and my Required Forms. I understand that I must accurately complete all forms required by iD Tech and any state or location provider where required (including, but not limited to the health form) within the timeframes set forth by iD Tech. I understand that as a condition of enrollment, I as my student’s parent/guardian, will need to log in to My Account to fill out the Required Forms between March 1st and May 15th, of the year in which my student is registered to attend an iD Tech program. If I register my student after May 15th, the Required Forms will need to be filled out within 72 hours of registration. I understand that if I do not accurately complete all forms by May 15th, of the year in which my student will be attending an iD Tech program, that I may be charged a minimum twenty-five dollar ($25) processing fee. If I do not complete the Required Forms at least one week (7 days) prior to the start of my student’s course, my student’s registration in the course may be withdrawn and no refund will be issued. If my student needs to bring any medications to camp, I agree to fill out the necessary medication forms more than a week prior to the start date of my student’s program and to bring the medication to camp the first day. I agree to bring all medications in original containers that identify in English the prescribing physician (if a prescribed medication), the name of the medication, the dosage, any side effects, and the dispensing frequency. The information dosage and dispensing frequency on the bottle must match the information provided on the medication form. You agree that all medications, except inhalers and diabetic testing kits will be given to and dispensed by iD Tech staff. If your student will be bringing an epinephrine auto-injector (i.e, Epipen), you will need to bring two non-expired auto-injectors the first day of their program. One will be held by staff. The other will be held by the student. This is best practice having two auto-injectors on hand. If a student of any age is in anaphylactic distress, staff will immediately administer an injection and call 911. Your student will be required to self administer all other injectable medications under the observation of staff. If bringing glucose pens or standard syringes, we require you to bring a Sharps biohazard disposal container as well. At the end of your student’s program, you are responsible for retrieving all medications and containers from staff. I understand that staff cannot give my student over-the-counter medication without written permission from me (parent/legal guardian) or their physician. Over-the-counter medication will be dispensed as directed on the medication’s box/container based on the age of the student. If the dosage differs from what is on the box/container, a prescribing doctor’s note must be provided with the medication. I agree that if my student’s medication is left at camp after my student’s camp session has ended, that iD Tech has the right to dispose of any medication left and shall not be responsible for any damages or liabilities related in any way to such medication, including, but not limited to, replacement cost of the medication and any damages resulting from participant’s failure to dispense such medication. It is against Federal Law to mail by any means, medication from one party to another party. I understand that iD Tech suggests that students be current on all vaccinations according to the age-appropriate recommended immunization schedules published by the US Centers for Disease Control and Prevention (CDC), and that some locations require students to provide up to date vaccination information or acceptable waiver. I further understand that if up to date vaccination information is required at the location where my student is registered, and I do not provide up to date vaccination information that my student’s registration may be canceled without a refund. I agree to obtain or provide proof that my student has already had any and all vaccinations required by the location in which my student is attending. I may provide a signed religious or medical vaccination exemption in lieu of obtaining the required vaccinations. I must provide two different emergency contacts in the completed online Health Safety Form found in My Account/Forms section prior to the start of camp. I understand that the emergency contacts should be at least 18+ years of age. For each contact, it is highly recommended that you provide two different phone numbers.. I understand that some states and countries require that additional health and safety forms be completed and submitted prior to my student attending camp, in their jurisdiction. I am responsible for making sure that if my student’s participation requires such forms that the forms are completed in their entirety, including obtaining a physician’s signature, where required. I UNDERSTAND THAT FOR MY STUDENT’S SAFETY, I MUST PROVIDE ID TECH WITH ACCURATE AND COMPLETE HEALTH, BEHAVIOR AND DIETARY ISSUES/RESTRICTIONS. I MUST ALSO COMPLETE ALL FORMS LOCATED IN MY ACCOUNT. FAILURE TO PROVIDE REQUIRED INFORMATION CAN RESULT IN CANCELLATION OF MY STUDENT’S REGISTRATION WITHOUT A REFUND.III. Special AccommodationsIf your student requires any special accommodations in order to participate in iD Tech activities, you must call iD Tech at 1-888-709-8324, no less than three weeks prior to your student’s first day of camp. iD Tech will work with you to identify available reasonable accommodations. If your student requires an aide at school then he/she will need to be accompanied by an aide at the iD Tech program, and that aide will need to be present for the duration of the session. The aide must be age 18 or older, may not be a family member, and must achieve a favorable result on a criminal background check. They may also be subject to fingerprinting. You will be responsible for all direct costs, including background check processing fees and compensation for the aide’s attendance.IV. At Camp ActivitiesField Trips: I give staff members permission to take my student to places of relevant interest, the swimming pool (where available), guided campus tours, and facilities within walking distance of camp, both on and off campus. Public and/or private transportation will be utilized.Photos: As a condition of participation, I authorize that photos, videos, images, audio and testimonials may be taken of me and my student and agree that said content may be used by iD Tech in promotional materials, marketing collateral, and online media. These images, testimonials, photos, videos, and audio may be shared and used by corporate partners, the media, or other organizations who work with iD Tech. I understand that iD Tech, its owners, agents, partners, facility providers, and employees will not be held liable for damages and injuries associated with said publishing, including any and all claims based on negligence. I agree that all images, testimonials, photos, video, and audio taken at or in connection with iD Tech are the sole and exclusive property of iD Tech.Press Visits: As a condition of participation, I authorize iD Tech and the press to use my student's full name and likeness in print, radio, TV and other mediums. (Often when press arrives at camp, iD Tech may not have the opportunity to contact all families for permission before the press arrives. Therefore, we have created a blanket policy. We thank you for understanding.)Projects/Take-homes: I understand that depending on the course my student takes, iD Tech will attempt to send my student home with a working game or project, or post my student’s project to a project website, and may send home a product or voucher for a product. However, I understand that there will be instances when a project or product or product voucher cannot be sent home, posted or delivered, and that iD Tech is not responsible if the game, project, product or voucher does not work properly, or are not compatible with systems outside of camp. Therefore, I release iD Tech from any responsibility for failure to provide a copy of the project or product voucher, or a non-functioning/non-compatible/non-complete games, projects, product vouchers or product. iD Tech staff will attempt to provide each participant with their completed project at the conclusion of each session and provide products or product vouchers (if applicable) at the end or any other time during the session. Refunds will not be issued for not receiving products, product vouchers, or being provided a copy of the project, and/or non-functioning/non-compatible/non-complete projects, product vouchers or products. I also agree that all projects and work created while at iD Tech may be used by iD Tech in promotional materials, online, and other print media and may be shared and used by corporate partners, the media, or other organizations who work with iD Tech. I agree that iD Tech, its owners, agents, partners, facility providers, and employees cannot be held liable for damages and injuries associated with said publishing, including any and all claims based on negligence. I understand that if I have issues with a product voucher or product, I must contact the manufacturer directly. I also understand that product vouchers only cover shipping within the continental US and that if I require the product to be shipped outside I am responsible for all shipping and handling costs.Movies: I understand that movies played at or attended by any of the programs run by iD Tech are rated PG and PG-13 and selected at the sole discretion of iD Tech.Software Accounts: I understand that some iD programs require creation or use of an online account during instructional time, or will require an online account to be created for access to games and networked gameplay. I give consent for iD Tech to create account(s) as needed for my student to participate in program activities. I understand that during non-instructional time, my student may have access to websites that require accounts to be set up. While it is against iD Tech rules for students to set up accounts without their instructor’s permission, there may be instances where a student may create an account without the knowledge of iD Tech or its employees. I hereby release iD Tech and its employees from any and all responsibility and liability for accounts created by my student without iD Tech’s knowledge.Game Ratings: I understand that while iD Tech takes its corporate responsibility and iD Tech family values very seriously, it cannot guarantee that younger students at iD Tech will avoid complete contact with the games rated "T" for Teen, or "M" for Mature. iD Tech will make a concerted effort to minimize exposure to any games not rated for my student’s age group. I also understand that if my student is attending a course designed for ages older than my student, some of the game titles my student may be exposed to may be rated for over my student’s age, and that if my student is attending a course for ages 13+, they may be exposed to games rated "M" for Mature by the ESRB (Entertainment Software Rating Board). In exchange for my student’s participation in iD Tech programs, I voluntarily assume any and all risks, known or unknown, associated with my student’s access to "M" rated game content at an iD Tech program.Inappropriate Web Content: I understand that iD Tech makes every reasonable effort to prevent participants from being exposed to inappropriate web content. As a safeguard, iD Tech will attempt to either install Internet filtering software on iD Tech computers, or use an outside filtering vendor. I agree that no method is 100% perfect, and that my student may inadvertently access some inappropriate content even though iD Tech takes reasonable precautions to guard against inappropriate material being viewed. I understand that if inappropriate content is viewed, iD Tech staff will follow standard protocol and agree that if my student is found in violation of the Code of Conduct set out above, my student may be removed from a program without receiving a refund.V. Sign OutSelf Sign-Out Policy for students 13 years of age or older: I agree that if my student is 13 years or older by December 31st of the year in which they are registered for a course, then he/she can sign himself/herself out with staff after 5 PM Monday through Thursday and after 3, on Friday, unless attending an overnight program. Once my student signs themselves out, they will no longer be in the care of iD Tech. Students signed up for an Overnight program may only sign themselves out at 3 PM on Fridays. If my student needs to be checked out at times other than the standard pickup time, then I must contact the Camp Director to make arrangements.Sign-Out Policy for students under 13 years of age: I agree that if my student does not turn 13 by December 31st of the year in which they are registered for a course, he/she must be signed out of their iD Tech program by an authorized pickup person. I understand and agree that only those knowing the code word—or those named as additional authorized pick-up persons (and who show a valid picture ID, Driver License, or Passport)—will be able to pick up my student. I understand that if I grant my child permission in My Account to be cleared for "self sign-out", that once my student signs himself/herself out, he/she will no longer be in the care of iD Tech, and that they are subject to the same rules as set out in paragraph 1 of this Section V. If I choose to allow my student who is not yet 13 years old to self sign-out, I understand that the terms of paragraph 1 of this Section V will apply regardless of my student's age.VI. Age PolicyI understand that iD Tech offers programs designed for students ages 7-18 and 16-19, and that students who are age 18 or 19 may interact with students under the age of 18 throughout the duration of each program session, within the following span of ages; ages 7-18 at iD Tech Camps, ages 13-18 at Academies, and ages 16-19 at AcademyNEXT. I understand that by registering for an iD Tech program that accepts 18-year-old and 19-year-old students my student may interact and/or room with a student that is 18 years old. I also agree that if my student is 18 or 19 years old that he/she must successfully pass a criminal and sexual offender background check, prior to being allowed to attend an iD Tech program. I agree to be responsible for all costs and fees associated with any background checks required for my student to attend.VII. Course CancellationI agree that iD Tech has the right to cancel a course. If a course is cancelled, I understand that I may enroll my student in another course. I agree to be responsible for any costs for the new program that are above the amount paid for the original course. If another course is not available in the local vicinity for the same dates, a refund will be issued for all monies paid to iD Tech for the cancelled course. I understand and agree that if iD Tech is forced to close a course based on a recommendation by the local Health Services Department (in the case of a communicable disease outbreak, etc.) or due to other acts of nature, I will not be entitled to a refund.VIII. Release of LiabilityON BEHALF OF MY SON/DAUGHTER/WARD, I, THE PARENT/GUARDIAN, IN EXCHANGE FOR THE RIGHT OF MY SON/DAUGHTER/WARD TO PARTICIPATE IN ID TECH PROGRAM(S), HEREBY RELEASE INTERNALDRIVE, INC., ITS OWNERS, AGENTS, PARTNERS, FACILITY PROVIDERS AND EMPLOYEES FROM LIABILITY (INCLUDING CLAIMS BASED UPON NEGLIGENCE) FOR ANY AND ALL DAMAGES OR INJURIES TO MY SON/DAUGHTER/WARD OR DAMAGE OF ANY PERSONAL PROPERTY. I AGREE TO BE FULLY RESPONSIBLE FOR ANY AND ALL SUCH DAMAGES OR INJURIES WHICH MAY RESULT DIRECTLY OR INDIRECTLY FROM ANY NEGLIGENT ACTS OR ACTIVITIES ASSOCIATED WITH INTERNALDRIVE, INC. HOWEVER, I UNDERSTAND THAT I AM NOT RELEASING INTERNALDRIVE, INC., ITS OWNERS, AGENTS, PARTNERS, FACILITY PROVIDERS, AND EMPLOYEES FROM GROSS NEGLIGENCE, RECKLESS CONDUCT OR INTENTIONALLY TORTIOUS CONDUCT. TO THE EXTENT THIS RELEASE CONFLICTS WITH STATE/PROVINCIAL LAW GOVERNING RELEASES, THIS RELEASE IS TO BE GIVEN THE FULLEST FORCE AND EFFECT PERMITTED UNDER STATE/PROVINCIAL LAW. SHOULD ANY PART OF THIS CONTRACT BE FOUND INVALID OR NOT ENFORCEABLE BY A COURT OF LAW, THEN THE REMAINING PORTION SHALL CONTINUE TO BE VALID AND IN FORCE.IX. IndemnificationI agree to defend, indemnify, and hold internalDrive Inc. /iD Tech, its officers, directors, employees, and agents, harmless from and against any claims, liabilities, damages, losses, and expenses, including, without limitation, reasonable attorneys' fees and costs, arising out of or in any way connected with my student’s participation in an iD Tech program.X. Arbitration AgreementI agree that any dispute arising out of or relating to this Agreement, my student's participation at INTERNALDRIVE, INC., or otherwise arising between the parties, including, without limitation, any students statutorily created or protected rights, as permitted by applicable state/provincial or federal laws, may be settled by arbitration in the state/province and county in which the camp is held, in accordance with the Commercial Rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrator(s) may be entered in any court of competent jurisdiction. The prevailing party in the arbitration shall be entitled to recover expenses including costs and reasonable attorney's fees associated therewith. Should any part of this contract be found invalid or not enforceable by a court of law, then the remaining portion shall continue to be valid and in force. I hereby acknowledge that I understand the terms of this ARBITRATION AGREEMENT, and agree to comply with all of its terms and provisions.XI. Payment PolicyUS Dollars: I agree that all financial transactions are made and quoted in U.S. Dollars.Non-Refundable Fees: To cover the administrative costs associated with registering my student, I agree that for each iD Tech program/course (other than iD Tech Academy and AcademyNEXT courses) that my student is registered in, $250 per student, per course of the total payment amount is non-refundable and non-transferable. If my student is enrolled in an AcademyNEXT, $1000 per student of the total payment will be non-refundable and non-transferable. If my student is enrolled in an iD Tech Academy course, $500 per student, per course of the total payment will be non-refundable and non-transferable. I also agree that iD Tech may impose a $40 per student per session Safety and Compliance Fee that is also due at the time of registration and is non-refundable.Late Fees: If I do not pay the total cost of my student’s program(s) at the time of registration, or through a payment plan, I understand and agree that I must pay the total amount due on or before May 2nd of the year of the program registered. If the total balance is not received by May 2nd, I understand and agree that I will pay a $25 late fee; If any balance remains on my account as of May 22nd, of the year of the program registered, I agree to pay an additional $25 late fee (for a total of $50). If my full balance is not paid within 2 weeks (14 days) of my student’s course start date, I agree that my student’s registration will be cancelled without a refund. I also understand and agree that if any balance remains outstanding 30 days after the last day of the last session my child attends camp in any given year, iD Tech has the right to charge a 1% monthly finance charge on such balance and send such balance to a collection agency for collection (collection fees may apply).Credit Card Authorization: If I make my initial payment by credit card and My Account has a balance remaining on May 2nd,of the year of the program my student is registered, to avoid incursion of late fees, I authorize iD Tech to charge the remaining balance due to the credit card on record.Payment Plan: By selecting to participate in iD Tech’s automatic payment plan, you agree to allow iD Tech to withdraw, each month, the amounts as designated by iD Tech, from the credit card on file. iD Tech does not charge anything to participate in this program, nor do they charge any fees should a payment not be honored on the date designated for withdraw. If a payment is returned, iD Tech will attempt to run the payment through two additional times, the first one week after the initial due date and the second, a week after the first retry. If the payment does not go through during any of the three attempts, you will be notified and will be removed from the automatic payment program, at which time all remaining balances will become due and subject to late fees as set forth above.Returned Check/Dishonored Transaction Fees: A $25 returned check fee will be assessed for any checks returned or card transactions that are not honored.Tuition: Tuition includes all state/province and federal taxes (except for VAT, which will be added to the course cost, fees and add-ons if applicable), adult supervision, instruction, curriculum materials, camp T-shirt, and recreational activities. Overnight Program Tuition includes daily meals (Sunday dinner - Friday lunch), double-occupancy dorm room (subject to availability), 24-hour adult supervision, and recreation.XII. Changes, Cancellations and Refunds24 Hour Cancelation: I understand that I have the right to cancel a reservation within 24 hours of the reservation being made. If the cancelation is made within 24 hours of the reservation, I will receive a full refund of all amounts paid for that reservation, including deposit amounts.Changes Made More Than 14 days Prior to Session Start Date: I understand and agree that I am, without penalty, able to make changes to my student’s registration including but not limited to location, date, overnight, weekends or course, so long as the changes are made more than 2 weeks (14 days) prior to the start date of the originally scheduled course my student was registered for. I understand that I will be responsible for any increased charges.Changes Made 14 days or Less Prior to Session Start Date: I understand and agree that if I change my student’s registration within or less than 2 weeks (14 days) prior to the start date of the course for which my student is registered, I will be responsible to pay a change fee of $250 per iD Tech and Alexa Cafe course, $500 per Academy course, and $1000 for AcademyNEXT, as well as any increase in charges based on the change. I must make my changes through My Account or an email to info@iDTech.com (please note your “Change” in the subject line). NOTE: If a medical emergency occurs less than 2 weeks (14 days) prior to the start of the course in which your student is registered, preventing your student from attending the course, please call 1-888-709-8324 and we will work with you to try to get your student registered in another course.Cancellation: I understand and agree that I must make my changes and cancellation through My Account or an email to info@iDTech.com (please note “Cancellation” in the subject line). If I cancel my student’s registration more than 2 weeks (14 days) prior to the start date of the course for which my student is registered, I will receive a refund as set out in Paragraph 4 below. If my student cannot attend a session for which they are registered and I do not notify iD Tech more than 2 weeks (14 days) prior to the course session date, it will be considered a withdrawal/no show and I will be responsible for full payment of all fees associated with my student’s registration for that course.Refunds: If I cancel my student’s registration through My Account or email as set forth above, more than 2 weeks prior to the start date the session I am cancelling, I understand and agree that the amount I paid for the cancelled registration, less the nonrefundable fees as set out in Section XI.2., will be refunded to me. I agree that refunds will NOT be issued for withdrawals/no shows or cancellations made less than 2 weeks (14 days) prior to the start date of the session cancelled or changed, and improperly made cancellations. I also understand and agree that refunds will NOT be issued for early withdrawal or dismissal due to absences (for any reason) including sickness, behavioral dismissals, power failures, weather, and unforeseeable events not within the control of iD Tech or other acts of nature. If students leave a program early for any reason, the tuition is not prorated and no refund will be issued, including partial refunds. Refunds will NOT be given at the iD Tech Academies in the event that a studio tour is cancelled. If iD Tech must cancel a session and cannot find a suitable alternative at an adjacent location, a full refund will be made to the client.XIII. Promotional SpecialsI understand that if I use a promotional discount when registering my student, that I am limited to one discount per student, and that the balance may need to be paid in full at the time of registration to receive the published discount amount. I also agree that all applicable cancellation/refund policies apply, and that all promotional codes must be submitted at the time of registration. I understand that iD Tech will not honor retroactive adjustments and that the total discounts received cannot exceed the total cost of the products purchased.XIV. Multi-Course DiscountIf I register and pay for more than one student or register one student for multiple iD Tech programs/courses for the same season, I understand that I will be responsible for paying the standard tuition for the first program/course, but will receive a discount of $50 for each additional program/course tuition paid. I understand that the lowest discount amount will be applied if I qualify for multiple discounts.XV. Refer-a-Friend ProgramIf I choose to participate in the Refer-a-Friend program, I do so voluntarily and understand and agree to the following:My referral code expires on June 1st of the current camp season and can only be used 10 times. For each new student that registers for any iD Tech program/course using my referral code and attends the course for which they registered, I will receive a $50 iD Tech course tuition credit which I must use that same season. My friend will also save $50 off their course tuition. The referred student must be a new student and cannot have previously attended any iD Tech programs. Referral codes must be used at the time of registration and payment and cannot be retroactively applied. Referral codes can only be used once per student. I understand that iD Tech will credit the referral based off of the referral code that the new client enters at the time of registration. The Refer-a-Friend program does not apply to siblings (since they are eligible for the Multi-Course Discount). Students may not refer each other to both qualify for the Refer-a-Friend Discount. The tuition credit will be applied within 4-6 weeks from the time the referred client registers and pays in full, or sooner. If the referred friend cancels his/her session, the credit will be removed from My Account and I will be responsible for any account balance that is created as a result of the lost credit. All tuition credits must be used in the season in which they are earned, can be used for course tuition including meals, CEUs, and other course fees, and will not entitle me to any form of payment. If I cancel or change the registration of a course in which I used a Refer-a-Friend course tuition credit, I am subject to the terms and conditions set out in Section XII above, and will only receive a refund of amounts paid directly by me. I will not receive a refund of tuition credit amounts.XVI. Camp Certificates/VouchersAll certificates/vouchers are non-refundable, non-transferable and not redeemable for cash. Certificates/vouchers must be redeemed at the time of registration. Location and date is based on availability at the time of registration. Certificates/vouchers are valid until the specified expiration date, without exception. They are valid for up to the amount issued, and any amounts not used are forfeited.XVII. Spring Break CampTo reserve space for Spring Break Camp, all tuition and fees for each participant are due in full at the time of registration. Partial payments are not accepted.I understand that after registering, I will receive a confirmation that includes information necessary for me to access My Account and my Required Forms. I understand that I must accurately complete all forms required by iD Tech and any state or location provider where required (including, but not limited to the health form) within the timeframes set forth by iD Tech. I understand that as a condition of enrollment, I, as my student’s parent/guardian, will need to log in to My Account to fill out the Required Forms before April 1st of the year in which my student is attending Spring Break Camp. If I register my student after April 1st, the Required Forms will need to be filled out within 72 hours of registration. I understand that if I do not accurately complete all forms by April 1st, of the year in which my student will be attending an iD Tech program, that I may be charged a minimum twenty-five dollar ($25) processing fee. If I do not complete the Required Forms at least one week (7 days) prior to the start of my student’s course, my student’s registration in the course will be withdrawn and no refund will be issued.I understand that I may change registration to another iD Tech program until 72 hours before the Spring Break Camp session start date, without penalty. If a change is made, the tuition paid for Spring Break Camp will be applied to the new program (excluding Tech Rocket) occurring in the same calendar year. I agree to be responsible for any additional tuition fees incurred due to the change.I understand that refunds will NOT be issued for early withdrawal or dismissal due to absences (for any reason including sickness), behavioral dismissals, power failures, weather, and unforeseeable events not within the control of iD Tech, or other acts of nature. If my student leaves Spring Break Camp early for ANY reason, the tuition will NOT be prorated and no refund will be issued or transferred.XIX. Contact with camp Instructors/StaffUnder no circumstances should a student give their personal contact information to staff, nor is staff allowed to give students their personal contact information. Staff may exchange personal contact information only with parents/guardians, if they choose to do so after receiving a request from the parent/guardian. All contact outside iD Tech programs between students and staff must be arranged by the student’s parent/guardian through communication directly with the staff. Should parents/guardians choose to engage with staff, or choose to let their student contact staff outside of iD Tech programs, they do so at their own risk.XX. iD Tech OnlineIf I am purchasing an iD Tech Online program, I have read, understood and agree that I and my student are also bound by the iD Tech Online Terms and Conditions found HERE and the privacy policy found HERE.XXI. Rights ReservedinternalDrive, Inc. reserves the right to update or modify these Terms and Conditions at any time. iD Tech is not a university-sponsored event. Activities, guest speakers, and tech tours will change session to session and may not apply to all locations. We reserve the right to cancel a course if participation numbers are not adequate. In the case where a suitable iD Tech replacement week is not found, a full refund will be issued. On rare occasions, the software and hardware we use at our programs may change without notice, and we will strive to find suitable software and hardware alternatives.XXII. State NoticesMassachusetts Required Legal Statement:"This camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the local Board of Health. I agree to uphold the copyright and trademark rights of iD Tech and their partners.Location SpecificTerms ConditionsThe parties to this Agreement are the student attending an iD Tech Program at Stanford University (Participant), Participant's parents or legal guardian, if Participant is under 18, (all referred to hereafter jointly and severally as "Participant") and the Board of Trustees of the Leland Stanford Junior University its officers, trustees, faculty, agents, representatives, volunteers, students and employees (collectively referred to hereafter as "Stanford”) for iD Tech programs held at Stanford University (“Event”) (“Stanford” and “Event” are collectively referred to hereafter as “Released Parties”).Assumption of Risk. Participant is a voluntary participant in this Event. Participant understands and agrees that the Event and any related activities may be dangerous, may involve travel, and that neither the Event nor Stanford can guarantee the safety of Participant. Participant is responsible for researching and evaluating the risks he/she may face and is responsible for his/her actions. Any activities in which Participant may take part, whether as a component of the Event or separate from it, have been undertaken with Participant's understanding and acceptance of any and all risks involved, which include but are not limited to physical or psychological injury, pain, suffering, disfigurement, temporary or permanent disability, economic or emotional loss, property loss or damage, loss of income or career opportunities, and/or death. Participant understands that these injuries or outcomes may arise from his/her own or others’ actions, inaction, or negligence; conditions related to travel; or the condition of the location where the Event is taking place. Nonetheless, Participant assumes all related risks, both known or unknown, whether or not listed above, of his/her participation in the Event, including travel to, from and during the Event.Physical Condition and Insurance. Participant attests that she/he is physically and mentally capable of participating and has no known health or other restrictions that might jeopardize her/his safety or health or the safety or health of others during their participation in the Event. Participant gives permission for Stanford or its representative to provide immediate and reasonable emergency care should it be required. Participant agrees that Stanford may record, edit, use, reproduce, publish and distribute by way of any and all media and transmission, the visual and/or audio likeness of Participant and other commentaries, information, and materials the Participant may provide in connection with the Event, which includes, without limitation, Participant’s name, biographical information, recorded voice, likeness, commentaries, presentation materials, and/or performance at the Event. Stanford is further granted permission to use such materials for educational, fund-raising, promotional or other purposes worldwide and in perpetuity Participant agrees that Stanford will be held harmless from any liability that may arise regarding the production, use, and distribution of such materials as described herein, and Stanford is hereby released from any claims relating to the rights granted above.Waiver and Release of Claims. In consideration of being accepted into and/or participating in the Event, Participant agrees to and hereby does, for Participant and on behalf of Participant’s heirs, executors, administrators, employers, agents, representatives, insurers, and attorneys, release and discharge Stanford of and from any and all claims which may arise from any cause whatsoever, including claims arising from any negligent act or omission by Stanford or others. However, I acknowledge that there are certain risks of physical injury or illness associated with the Activity.In return for My Child's participation in the Activity: I fully and forever RELEASE, WAIVE, DISCHARGE, ACQUIT, INDEMNIFY, HOLD HARMLESS and COVENANT NOT TO SUE, Washington University in St. Louis, including its governing board, officers, employees, students, agents and volunteers (hereinafter collectively referred to as "Releasees") from any and all liabilities, claims, or injuries, including death, that may be sustained while participating in this activity, including but not limited to travel to, from, and for the activity, or while on premises owned or controlled by Releasees. I understand this release does not apply to injuries caused by intentional or grossly negligent conduct on the part of the Releasees. This includes counselors, organizers, and any program personnel residing and visiting in the residence halls. The cooperation of everyone is required if summer programs are to be successful and rewarding for all participants. The effective date and time of this agreement is equivalent to the date and time you agree to these Terms and Conditions.Thank you for your cooperation.University of MichiganPLEASE READ BELOW:RESIDENCE HALL RULES AND REGULATIONSCONDUCTActs of discrimination or discriminatory harassment based upon an individual’s race, sex, color, religion, creed, national origin or ancestry, age, marital status, handicap, Vietnam-era status, or sexual orientation are a serious violation of University policy and will be dealt with promptly. Sexual harassment is illegal under both state and federal law. In some cases, it may be susceptible to prosecution under the criminal sexual conduct law.Alcoholic beverages, illegal drugs, laser pens, fireworks, explosives, and all weapons are absolutely prohibited in the hall. It is against the law to tamper or misuse building fire alarm or fire-fighting equipment. Tampering with or misuse of elevator alarms, emergency call buttons or calling 911 from a room or elevator telephone, except in an emergency, is against the law. Penalties will be enforced.Each room will be inspected prior to arrival and before departure and any damages to the room or to University property will be assessed and charged to the program, which may then be assessed to the participant. The room must be left in the same condition as it was during move in. Do not fasten anything to the walls or tamper with window screens. Excessive mess resulting in extra housekeeping hours will result in a charge to the program, and may be passed on to the student.Appropriate behavior is expected at all times. Engaging in sexual contact with another student is a violation of our conduct policy. Leaving the program or residence hall without permission and appropriate supervision is a violation of our conduct policy. Bullying, taunting or threatening behavior toward other students is a violation of our conduct policy. Excessive noise or rowdiness will not be tolerated. Dangerous behavior, including fighting, is a violation and will not be tolerated. Throwing items out of residence hall windows is a conduct violation.The use of candles, open flames, explosives, smoke bombs, “snaps”, or fireworks is dangerous and will not be tolerated in the residence halls, or at any U-M facility.WE ARE A SMOKE FREE CAMPUS. SMOKING ON CAMPUS OR IN OUR BUILDINGS IS PROHIBITED.AGE ELIGIBILITYAll students residing in a University of Michigan residence hall must be ten (10) years of age on or before the start of their program. There are no exceptions to this policy.MEDICATIONSAll medications prescribed by a doctor must be dispensed by program nurse or counselors. Please fill out the medical forms and bring medications to the attention of the program staff at registration. Students may only be checked out of the residence hall by a parent/guardian or program staff. Visitors, other than parents, who may be taking the student from the residence hall, must be approved by the parent/guardian during registration.KEYS AND STUDENT IDENTIFICATIONStudents are identified in a variety of ways including room keys and lanyards. Some programs may use wristbands. Any identification specific to the program will be issued along with a room keycard and lanyard at check-in. If the identification is lost, a replacement may be obtained at the front desk or from a staff member. The room keycard and identification should not be loaned to anyone and are not transferable.MEAL TIMESYou may eat at any time during the hours your program has designated for meals. Students may be asked to show program identification at mealtime. In the case of lost identification, help is always available from dining and program staff. Meal hours will be provided by your program staff and are available from residence hall staff as well. Students are expected to eat meals in their assigned dining facility during their designated meal times. Food allergies should be reported to the program and dining staff prior to the student’s arrival.TELEPHONESLand lines are being phased out of residence halls due to the prevailing use of cell phones by our students. Students may place collect and credit card calls from their room phone (if available) and receive long distance calls. Counselors will assist students with urgent telephone needs.MOTOR VEHICLESStudents are not to bring private automobiles or motorcycles to the University. If it is necessary for a student to drive, please discuss permission and parking with the program organizers. The vehicle must be registered and the keys turned in to the program organizers/counselors during the entire program. The University assumes no responsibility for a stored vehicle on University property.PERSONAL PROPERTYThe University assumes no responsibility for personal property. Please leave valuable electronic items at home. Bring only a small amount of personal expense money with you since the halls do not have any secured storage facilities. You may wish to consult with your insurance company regarding coverage of your personal property while away at program.FOR SECURITY, ROOM DOORS ARE TO BE LOCKED AT ALL TIMES.DAMAGESRoom checks will be conducted by program staff to determine room damages. Refrigerators are not available through the Residence Hall.EARLY DEPARTURESIf a student must leave before the official ending date for the program, a parent or guardian must notify the program organizers, and fill out any necessary forms with the program. If the student is being picked up by someone other than the person designated parent/guardian, written permission naming the responsible adult must be placed on the back of the registration card, during registration, prior to the student being released. A signed Early Release form from the program organizer must be turned in upon checking out from the resident hall. Identification is required.PENALTIESVIOLATION OF THESE RULES AND REGULATIONS MAY RESULT IN DISCIPLINE, UP TO AND INCLUDING IMMEDIATE DISMISSAL FROM THE PROGRAM AND SUBSEQUENT U-M PROGRAMS.Students are reminded about proper behavior upon their arrival. Inappropriate behavior will be pointed out and verbal warnings will be given to students by organizers and counselors to correct the behavior. I give my permission for my child to be on the campus of the California Institute of Technology (“Caltech”) as part of the iD Tech Camp on the dates for which I registered. The activities of the iD Tech Camp include computer programming and application design; recreational activities. I understand that the iD Tech Camp is a separate entity from, and in no way related to, Caltech. I understand that iD Tech Camp has simply contracted with Caltech to hold its camp on the Caltech campus, and that Caltech has no involvement in the camp, including but not limited to, no supervision or teaching of my child. I understand that my child’s participation in iD Tech Camp is completely voluntary. I further understand that there are inherent risks of injury, serious bodily harm, or death associated with the iD Tech Camp. In particular, the risks of iD Tech Camp include but are not limited to, personal physical injury, equipment malfunction and/or failure including, damage to property, actions of other people, and other injuries arising out of or connected with iD Tech Camp. I expressly agree to accept and assume all such risks, including personal injury and death, arising in any way from my child’s participation in iD Tech Camp. I represent that my child has no physical or mental condition that prevents him/her from participating in iD Tech Camp in a manner that is safe for my child and others, and acknowledge that it is my responsibility to take all appropriate actions in advance of my child’s participation in iD Tech Camp. I further acknowledge and agree that I have the responsibility to consult with my child’s physician to determine if medical conditions exist that would pose a direct threat to my child’s health or safety or the health or safety of others. In consideration of the benefit my child will receive from participating in iD Tech Camp, I hereby agree, on behalf of my child, myself and on behalf of my child’s and my assignees, descendants, dependents, heirs, next of kin, distributees, parents, guardians, executors, administrators, successors, estate, and legal or personal representatives, to release and discharge and promise not to sue Caltech and any subsidiary or affiliate or government sponsor of Caltech (collectively referred to as “Caltech”), as well as any person acting in his/her capacity as employee, officer, trustee, agent, contractor, or representative of Caltech (collectively referred to as “Released Parties”), from and with respect to any and all claims, demands, actions, suits, causes of action, and liabilities of whatever kind or nature in law, equity or otherwise, that may arise from, are related to, or are in any way connected with iD Tech Camp, including injury, death, damage or loss, whether it results from the negligence of Caltech and/or any other Released Parties, or from any other cause, provided, however, that this does not extend to gross negligence, willful misconduct or a violation of law by Caltech or any other Released Parties. I knowingly and voluntarily waive any and all rights and benefits conferred upon me by the provisions of Section 1542 of the California Civil Code or by any similar law or provision, which Section reads as follows: “A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS WHICH THE CREDITOR DOES NOT KNOW OR SUSPECT TO EXIST IN HIS OR HER FAVOR AT THE TIME OF EXECUTING THE RELEASE, WHICH IF KNOWN BY HIM OR HER MUST HAVE MATERIALLY AFFECTED HIS OR HER SETTLEMENT WITH THE DEBTOR.” I understand and agree that if a claim, suit, or attachment is brought or sought against me as a result in any way of my child’s participation in iD Tech Camp, I shall not be entitled to any defense or indemnification by Caltech or the Released Parties in connection with such claim, suit, or attachment. I agree to indemnify Caltech and the Released Parties for any and all claims and liabilities against them arising out of or relating to my child’s participation in iD Tech Camp. In case of an emergency and if I cannot be reached I, the undersigned parent or guardian of the above named child, do hereby consent to any x-ray, anesthetic, medical, or surgical diagnosis or treatment and hospital care deemed advisable and rendered by any licensed physician or surgeon. This authorization is given in advance of any required care to empower a representative or other official of Caltech or iD Tech Camp to give consent for such treatment as the physician may deem advisable. This authorization is effective unless revoked in writing. I accept full responsibility for any medical expenses incurred as a result of these actions. I agree that Caltech has the right to make, use, create derivative works, and/or display and photos, video and/or audio tape recordings of my child. I understand that my child may be photographed and/or video or audio taped verbatim and that Caltech may allow persons external to Caltech to view the pictures or recordings in part or in their entirety. I am fully aware and agree that such use of my child’s image may include posting on publicly available internet sites, including Caltech sites and other publicly viewable social media sites. I waive any right that I may have to review or approve of any finished products, or the uses to which such products may be applied. I release and discharge Caltech and the Released Parties from any liability to me by virtue of any representation that may occur in the creation or use of said photos and/or video or audio tape recordings. I represent and warrant that I have the full right, power and authority to enter into and execute this Release and to grant all rights granted under it. I am over the age of 18, have read and understand the terms below, and am voluntarily allowing the participant to participate in the Conference at Loyola. I hereby warrant and represent that my student is physically fit and does not have any medical condition or physical limitations that would put them at risk for injury as a result of my participation in the Conference. In consideration of my student’s voluntary participation in the Conference and use of Loyola’s facilities, (i) I hereby assume all risks of injury which may result from my student’s participation in the Conference and use of Loyola’s facilities, and (ii) I agree, on behalf of myself, my student and our heirs, executors, administrators, and assigns, to waive, indemnify, hold harmless, release and discharge Loyola, its affiliates, including Mundelein College, and their respective officers, trustees, directors, employees, agents, successors and assigns from any and all claims, damages, demands, rights of action or causes of action, present or future, known or unknown, anticipated or unanticipated, resulting from, or arising out of, my student’s participation in the Conference, and use of Loyola’s facilities. I, and my student agree to abide by all of the rules and regulations of Loyola in effect during my student's participation in the Conference. I understand that Loyola is not affiliated with the Organization and that Loyola is not a sponsor or co-sponsor of the Conference, but is merely allowing the Organization to use its facilities for the Conference. I have read and understand the foregoing and affirm that my student is participating in the Conference of his/her own free will.This Release is made on the undersigned date by and between the undersigned (or if a child under 18 years of age, on behalf of the child by the child's parent or legal guardian) and the Town of Westlake/Westlake Academy. The parties hereto agree and covenant as follows:The undersigned hereby fully releases, indemnifies, and holds harmless the Town, its elected officials, agents, employees and all others associated with the Town of Westlake/Westlake Academy, in both their public and private capacities, from any and all claims, demands, judgments, damages, expenses, attorney's fees, actions and causes of action arising out of or in any way connected with the participation in any activities connected with the use of the Town's facilities. This provision applies to any and all programs, facilities and activities, to the fullest extent permitted by law. The undersigned acknowledges that by signing this Release, he/she has read and understands this Release, that he/she has signed this Release of his/her own free will and not under duress or coercion, and that he/she has the authority to execute this Release. The undersigned acknowledges and fully understands that all activities involve risk, and the undersigned agrees to fully assume responsibility for any and all risks including, but not limited to, physical injury, death, and loss or damage of personal property. Further, the Town assumes no liability of any kind and is not responsible for the actions of other individuals or participants at its facilities. The undersigned acknowledges and fully understands that this Release, and all provisions contained therein is fully applicable, in its entirety, to guests, children, or any third party present as a result of the undersigned's presence or participation of any activity held at Town facilities. The terms of this Release are contractual and not a mere recital. Further, this Release shall be permanently binding upon the undersigned, his/her agents, successors, third parties, and legal representatives.THE UNDERSIGNED DOES HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS THE INDEMNIFIED PARTIES FROM ANY AND ALL LIABILITY AND SHALL DEFEND OR PAY JUDGMENTS AGAINST IT AND/OR THE INDEMNIFIED PARTIES, FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS OR CAUSES OF ACTION, INCLUDING REASONABLE AND NECESSARY COSTS, INCLUDING ATTORNEY'S FEES INCURRED IN DEFENSE OF ANY SUCH CLAIM, WHICH ANY OTHER PERSON, FIRM, OR CORPORATION HAS OR MAY HAVE ARISING OUT OF/OR RESULTING FROM THE PARTICIPATION IN THE METROPLEX SELECT ATHLETIC PROGRAM REFERENCED HEREIN, OR IN ANY WAY CONNECTED THERETO.I/We warrant that the undersigned is the parent or legal guardian of the child referenced herein and that I/We have the legal authority to enter into this Agreement and further that the undersigned have read this Agreement and fully understand all provisions referenced herein.I recognize that my child/ward (herein referred to as “Participant”) will be hosted by Conference Services at San José State University (herein referred to as “University”) as a part of the iD Tech Program (herein referred to as “Group”).I recognize that by signing this agreement I acknowledge my commitment to support the Participant’s involvement with this Group and with the University. I understand the Participant’s involvement with this Group and with the University, and I am aware of the following responsibilities of the Participant:University Policies. I/we understand that the Participant must abide by University policies, including the Conference Services Guest Regulations and Minor Regulations, which can be provided by your Group leader(s). The Participant must remain under supervision by Group staff/chaperone at all times while on campus. The Participant may be subject to disciplinary action, potentially as severe as removal from campus, for violations of those policies.University Setting. I/we understand that the University, as a public college campus in the United States of America, is host to many traditional public forums, which are places that by long tradition have been used by the public at large for the free exchange of ideas. These traditional public informs, including but not limited to outdoor areas (e.g. sidewalks, lawns, etc.), are open to the public, and the University cannot restrict access to or use of the areas by anyone (unless a compelling governmental interest is presented to limit time, place, or manner of the access/use).Residence Halls. I/we understand that the residence halls that the Participant will be living in are secure, non public forums and access to these facilities is limited only to approved staff and guests (i.e. others living and/or working in these facilities). Participants are expected to stay overnight in the accommodations arranged for them by the University. Specific room and housing assignments are made at the discretion of the Group and the University.Tobacco Free Campus. I/we understand that in accordance with Presidential Directive 2014-01, smoking and tobacco products are prohibited in all indoor and outdoor areas on campus, including vehicles and parking lots. The sale of tobacco products is prohibited, as is tobacco related advertising and sponsorship. More information regarding the Tobacco Free campus policy can be found at http://www.sjsu.edu/tobaccofree/.Removal from Campus. I/we understand that if the Participant disrupts the Group’s planned activities or the Participant's behavior gives the Group leader(s) reasonable cause to believe that the Participant’s continued presence with the Group poses a danger to the health or safety of persons or property, or impedes, disrupts or obstructs the Group’s or University’s functions in any way, The Participant may be removed from the Group program and from the University campus. Examples of grounds for immediate removal from campus include alcohol, drug and weapons related violations, assault, and sexual or racial harassment. Barring emergent need for want of safety, before the Participant may be removed from campus, The Participant will have an opportunity to explain their conduct to the Group leader(s). Any decision to remove the Participant from campus would be final. If the Participant is removed from campus for disciplinary reasons, I, as the Participant’s parent/guardian, will be responsible for their return home.Hygiene and Health. I/we understand that the Participant will be responsible for their own hygiene and health maintenance. In the event of serious illness, accident or emergency, the Participant will inform an appropriate Group official so that assistance may be secured and so that the Participant’s designated emergency contact may be notified. I agree to disclose the Participant’s pre-existing mental and/or physical health conditions in entirety on the waiver/release of liability provided to me by the Group. I/we understand that a situation arising from such a condition that has not been disclosed in advance which places the Participant, others, and/or the Group or University at risk may result in the Participant being removed from campus. I will check relevant agencies such as the Center for Disease Control and Prevention (CDC) and the World Health Organization (WHO) to check on required immunizations for the United States of America and the state of California. It is my responsibility and choice to obtain any required or recommended immunizations suggested by these agencies for the Participant in a timely manner prior to the Participant's participation with the Group.Governing Law. I/we understand that the Participant must obey all federal, state, and local laws that the University community is held to. Any dispute arising from this Agreement will be determined according to the law of the United States of America and the State of California.Program-Specific Additions to this Agreement. The Group may establish additional rules and policies regarding issues specific to that Group provided that they do not negate or contradict the policies of this Agreement or other Conference Services regulations and policies. I/we understand that the Participant will be responsible for understanding and abiding by any such additional rules while participating with the Group.Communication. I will respond within 24 hours to all calls from the Group leader(s) regarding the Participant’s participation with the Group. Failure to respond to calls in a timely manner may result in the Participant's removal from campus. Use of Photos, Quotes, and Other Materials. I/we authorize the use of materials pertaining to the Group’s functions to be used for marketing purposes by the University or the Group, including but not limited to photographs, quotes and other materials as deemed appropriate by the University or the Group.Please read this form carefully and be aware that in seeking permission from Northwestern University for your child to participate in a Northwestern University event, you will be waiving all claims for injuries you might sustain arising out of your participation. Participation in this event is voluntary and at your own discretion and judgement. As a participant in the iD Tech Summer Camp on for the summer of 2020, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, damages, or loss that I may sustain. Such risks inherent to this event may include: bodily injury.I do hereby fully release and discharge Northwestern University, its trustees, officers, agents and employees from any and all claims from injuries, damages or loss that may accrue to me on account of my participation in the iD Tech Summer Camp.I further agree to indemnify and hold harmless and defend Northwestern University, its officers, agents and employees from injuries, damages, and loss sustained by me and arising out of, connected with, or in any way associated with my participation in the iD Tech Summer Camp.I have fully read and understand the foregoing. It is my express intent that this agreement shall bind the members of my family, my heirs and assigns. I and my student understand that iD Tech reserves the right to cancel my student’s iD Tech Online product without any prior warning for violating any of the terms of the Code of Conduct. Refunds will not be given for products canceled for failure of the student or the parent to abide by the Code of Conduct. I also acknowledge that upon staff observation of my student during an iD Tech Online program, they may determine that the program is not a suitable and/or productive environment for my student. In this case, iD Tech may cancel all remaining programs and no refund will be issued. iD Tech strives to maintain excellent relationships with students. However, in some rare cases, iD Tech Online programs may not be a compatible environment for every student.Students and Parents/Guardians May NEVER: bully, intimidate, or harass others; use this service, or materials provided as part of this service for illegal or immoral activity; use inappropriate language; share lesson information with third-parties, without permission from instructor; impersonate another person; publicly display lesson(s); sexually harass others; display pornography or any inappropriate material (including sexual content, inappropriate violence, racism, bullying, etc.) during lessons; or personally contact instructors outside of lessons.Students and Parents/Guardians MUST:abide by the rules and regulations of the instructor and iD Tech; attend lessons in an appropriate, private setting; dress appropriately during lessons; adhere to the terms of use of any sites used, including following the specified age policies; and only share material that is related to lessons, and appropriate.II. iD Tech Online ProgramsAll Lessons are one hour sessions that will be provided between 6am and 9pm Pacific Time. Multi-lesson discounts can be purchased. However, multi-lesson purchases are intended to be used in full and unused lessons will not be refunded. All lesson credits must be used within one year of purchase. Lesson credits expire a year from purchase, with no refund. The fee for Online Private Lessons is non-refundable and lessons not used by the expiration date are forfeit, and cannot be refunded in full or prorated. Lessons can be rescheduled for any reason up to 24 hours prior to the scheduled lesson. No-shows or changes made within 24 hours of the appointment start time are credited against your available sessions. With at least 24 hour notice, we will gladly reschedule any appointment with no penalty. A post lesson progress report will be sent upon completion of each lesson. You authorize the program staff to attend to health issues or injuries your student may incur while attending the program. You acknowledge that you will be financially responsible for any medical treatment needed during the duration of the program. You give iD Tech staff members permission to take your son/daughter/ward to places of relevant interest, the swimming pool (where available), guided campus tours, and facilities within walking distance of the program, both on and off campus.You give permission for your student to go the places described below during his/her time at iD Tech. You understand that your student will be supervised by staff while on a walking trip.Occasionally, staff will take students to a location on and off campus to visit a local venue. This might involve going to another building on campus, a campus facility tour, a trip to a park, an outing to a local mall, or going to a movie theater. All events would be within walking distance of the program facilities.Important Note to Participants and Custodial Parents and Legal Guardians: By agreeing to these Terms and Conditions, you confirm that you understand and agree to the full content of the above. You understand that iD Tech does not in any way screen Members or review or police: (i) statements made by Members in their Member Submissions or the Member Submissions in general; or (ii) statements made by Users or any information a User may provide via the iD Site Services. You understand that your participant(s) is solely responsible for, and will exercise caution, discretion, common sense, and judgment in using the various iD Sites Services and disclosing personal information to other Members or Users. On behalf of your participant(s), you agree that they will take reasonable precautions in all interactions with other Members, particularly if they decide to meet a Member offline or in person. Your participant's use of the social networking sites with which iD Tech is affiliated, their services, and/or Content and Member Submissions, is at your sole risk and discretion and iD Tech hereby disclaims any and all liability to you or any third party relating thereto. On behalf of your participant(s), you agree that they will not harass, threaten, intimidate, bully, stalk, or invade the privacy of any individual in connection with your use of the social networking sites with which iD Tech is affiliated and their services, whether or not an individual is an iD Tech Member; and you further agree not to advocate such activities or to encourage others to engage in any such activities. On behalf of your participant(s), you agree they will not give their social networking information to an iD Tech staff member. You and your participant(s) should also be aware that under no circumstances are iD Tech employees allowed to give personal contact information for social networking sites.
