We truly believe in the involvement of mind, body, and spirit in life to remain vital and independent. We will continue to offer the support programs and information to assist you in any way we can.Watch for more fun and exciting events in 2019! Member Dues are $20 per year for those 55 and up and $25 for those under 55 or living outside of Monroe County. There are also many exciting volunteer opportunities available. Stop by for a tour and discover everything the Center has to offer! Hop on the treadmills or stationary bikes Enjoy a great breakfast or wonderful lunch. OPEN TO THE PUBLIC. Donations are also always welcome to support our programming that keeps seniors active. We are a worthy, mission-oriented organization that makes a difference in numerous lives of older adults, through wellness programs, socializing, nutrition, support resource advocacy and coordination, and problem solving.General contributions, funds designated for a specific purpose, in memory of a special person, or in-kind donations are gratefully accepted. We are a nonprofit 501(c)3 organization and all donations are tax deductible. We protect that trust by respecting privacy, even if our relationship ends. Types of Information We Collect: As a participant of the Center, we collect basic information - name, address, telephone numbers, date of birth, emergency contact information, and email address. For volunteers, we also conduct a background check, as well as a driving record check if driving on behalf of the Center. This information is deemed confidential by us and is securely kept. For members of specific groups, such as the 20 / 20 weight loss support group, individuals' weight and basic health status is shared within the group. This information is also confidential and kept within the group. Specific permission will be requested if we share the information outside the group or in reports. What Information is Shared: We are required to provide information about members to our funders, including the Monroe County Commission on Aging, to verify that services were indeed provided. This information includes name, address, date of birth, service date, and number of hours at the Center. Information may also be shared if there is a clear and immediate danger to a participant. Photographs: Photos are often taken at Center events and activities, which are used in the Center's newsletter and other publications. Participants are asked to please let us know if you they do not wish for your photo to be taken and/or used. How Else Information May be Shared: If participants request assistance from us to obtain services from another service provider, we will ask that a consent form be signed that specifically lists what information may be shared with the other organization on your behalf. Breach of Personally Identifiable Information: The Monroe Center for Healthy Aging values the safeguarding of personally identifiable information and has adopted a "Breach of Personally Identifiable Information" policy in accordance with the Identity Theft Protection Act (452) of 2004. With respect to one or more persons who are included in a database that discovers a security breach, notice of the security breach must be provided to each person included in the data base unless the person or Monroe Center for Healthy Aging determines that the security breach has not or is not likely to cause substantial loss or injury to, or result in identity theft.
