UPDATED 2022 Transition Coding and Payment Tip Sheet Available
Got Transition and the American Academy of Pediatrics have released their 2022 Transition Coding and Payment Tip Sheet, which includes new CPT Care Management codes, current Medicare fees and RVUs for the codes, and an insurance carrier letter that can be adapted for use to encourage payer recognition of transition-related codes.

Exciting New Playbook Helps School-Based Health Centers Assist Students with Health Care Transition
The School-Based Health Alliance and Got Transition have partnered to develop a "SBHC Playbook on HCT" that helps guide SBHC clinicians and other school staff to assist students with their transition from the SBHC to adult health care.

Toolkit Helps Prepare Young Adults with ID/DD and their Primary Care Team Prepare for an Emergency
A new toolkit from The National Alliance to Advance Adolescent Health, with funding support from WITH Foundation, assists primary care teams and their young adult patients with intellectual and developmental disabilities to plan for disasters and emergencies.

Webinar Recording: "Designing Value-Based Payment Innovations for Pediatric-to-Adult Transitional Care: Lessons from the Field"
The National Alliance to Advance Adolescent Health hosted a webinar on 1/26/22 discussing its new "Guide for Designing a Value-Based Payment Initiative for Pediatric-to-Adult Transitional Care" that featured senior officials from two large Medicaid MCOs who spoke about their own experiences with designing a transition VBP pilot.

Report Examines Changes in Implementation of HCT in State Title V Care Coordination Programs
The new Got Transition report shows how state Title V care coordination programs have made improvements in their use of evidence-informed HCT strategies over the past five years.
