Editor's Choice The nineteenth installment in our “Lung Cancer World Wide” series is Lung Cancer in France, by Dr Jean-Louis Pujol and colleagues. This manuscript is an important resource with an excellent overview of lung cancer screening, diagnosis and treatment in France. There is currently controversy over the best approach to the therapy of small cell lung cancer, as questions are raised about the utility and need for prophylactic cranial radiation and consolidative radiation in extensive stage small cell lung cancer. The American Radium Society through their Thoracic Appropriate Use Criteria Committee have developed updated consensus statements for the treatment of limited stage small cell lung cancer (American Radium Society™ Appropriate Use Criteria Radiation Therapy for Limited-stage Small Cell Lung Cancer 2020– Chung SG et al) and extensive stage small cell lung cancer ES-SCLC (American Radium Society Appropriate Use Criteria on Radiation Therapy for Extensive-stage Small Cell Lung Cancer – Higgins KA et al). These two manuscripts provide a useful overview of the current state of therapy for this disease.In the early days of anti-PDL1 therapy, atezolizumab was evaluated in the second line setting of NSCLC through the phase II POPLAR and phase III OAK studies of atezolizumab versus docetaxel. Both studies demonstrated a survival benefit. In this issue of the journal, Mazieres and colleagues present final overall survival and four-year survival data for both studies. Their report confirms the OS benefit with a longer OS observed in patients receiving atezolizumab vs docetaxel in POPLAR (median OS: 12.6 months vs 9.7 months; HR: 0.76) and OAK (median OS: 13.3 vs 9.8 months; HR: 0.78). Four-year OS rates in POPLAR are 14.8% and 8.1% for atezolizumab and docetaxel, respectively, and 15.5% and 8.7% in OAK. Key takeaways from these studies are that most 4-year survivors in the docetaxel arms received subsequent immunotherapy (POPLAR, 50%; OAK, 65%); most of these long term survivors had non-squamous NSCLC, a good performance status (ECOG PS 0) and approximately half had an objective response to therapy. Lung cancer stigma is known to negatively impact the clinical care and outcomes of patients. King and colleagues sought to determine whether attitudes about lung cancer had changed over a decade. The investigators used the same tools and methodology to perform a cross-sectional survey administered to the general public, oncologists, and lung cancer patients ten years apart (2008 and 2018). Their results, published in this issue of the journal, reflect recognition of treatment gains and increased visibility of lung cancer but also highlight that stigma still remains a significant problem, with an increase in stigma reported by patients.
