This concise narrative review provides a complex view of the diagnosis of paradoxical embolism and referring patients for patent foramen ovale closure. It aggregates current data from studies comparing the effectiveness of this surgical procedure and conservative solution and discusses the caveats, pros and cons of individual methods of paradoxical embolism prevention.

The high-performance technology CRISPR/Cas9 improves knowledge and management of acute myeloid leukemia Romeo Gabriel Mihaila, Diana Topircean

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):249-257|DOI:10.5507/bp.2021.048

CRISPR/Cas9 technology was used to develop new experimental models of acute myeloid leukemia for a better understanding of the pathogenetic mechanisms involved in leukemogenesis. The CRISPR/Cas9 system is used to understand drug resistance and find solutions to overcome it. The therapeutic progress achieved using the CRISPR/Cas9 system is remarkable and allows us to get closer to achieving the goal of curing acute myeloid leukemia in the coming decades.

KIR/HLA ligands immunogenetics markers associated with outcome of hepatitis B virus infection in the Bulgarian population Viktoria Varbanova, Georgi Popov, Veneta Grigorova, Diana Petrova, Elissaveta Naumova, Anastasiya Mihaylova

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):270-276|DOI:10.5507/bp.2020.043

This is the first study to associate NK KIR and KIR HLA ligand genetic polymorphisms with HBV susceptibility and course of disease in the Bulgarian population. Our data support the hypothesis that genetic factors could be associated with a predisposition to HBV chronicity or spontaneous recovery from infection which is ALSO reported by other investigators. Moreover, we found common protective KIR genotype for HBV infection and leukemia susceptibility in the Bulgarian population, supporting the suggestion of identical genetic factor regulating antiviral and antileukemic NK cell activity.

Nontuberculous mycobacterial infections in children in the Czech Republic in the period 2003-2018 Karolina Dolezalova, Marek Maly, Jiri Wallenfels, Dana Gopfertova

Long-term trends in the incidence, treatment, hospital fatality and subsequent mortality from acute myocardial infarction in the Czech Republic Jan Bruthans, Otto Mayer, Jiri Jarkovsky, Miroslav Zvolsky, Jan Bruthans jr

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):283-290|DOI:10.5507/bp.2020.014

The article documents a pronounced and continuous decrease in AMI incidence, fatality and long-term post AMI mortality on a national scale as the first such study in one of the countries of Central and Eastern Europe. The decrease in mortality of patients who were treated in hospital for AMI is very similar to the decrease in mortality of patients who actually did not receive any treatment for AMI - dying outside hospital or diagnosed AMI post mortem. We see this as proof of improved primary and secondary CHD prevention which document our data on cardiovascular prevention and treatments. However, the tremendous impact of progress in acute coronary care has to be acknowledged as well.

Regional differences in presentation characteristics, use of treatments and outcome of patients with cardiogenic shock: Results from multicenter, international registry Michal Pazdernik, Mario Gramegna, Allan Bohm, Maria Trepa, Christophe Vandenbriele, Salvatore De Rosa, Jamol Uzokov, Milica Aleksic, Milana Jarakovic, Mohammad El Tahlawi, Morsy Mostafa, Maria Stratinaki, Diego Araiza-Garaygordobil, Ekaterina Gubareva, Polina Duplyakova, Manuel Chacon-Diaz, Hesham Refaat, Federico Guerra, Alberto Maria Cappelletti, Vojtech Berka, Dirk Westermann, Benedikt Schrage

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):291-297|DOI:10.5507/bp.2021.046

The results of this large multi-national registry provide insights into regional differences of the characteristics, use of treatments and short-term mortality of patients with CS across the globe. Unique differences in presentation of CS were observed between European and Non-European centers, such as etiology of CS, age at presentation, gender prevalence, or use of vasopressors and MCS. Despite this marked heterogeneity, 30-day all-cause mortality did not differ between regions.

Difficult airway prediction in paediatric anaesthesia (Diffair): Prospective observational study Jozef Klucka, Martina Kosinova, Milan Kratochvil, Lukas Marecek, Petra Kovalcikova, Milan Urik, Petr Stourac

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):298-304|DOI:10.5507/bp.2020.020

The cumulative incidence of events associated with difficult airway in paediatric anaesthesia can be up to 10%, which is higher than previously reported. The efficacy of difficult airway prediction remains very low.

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):305-315|DOI:10.5507/bp.2020.041

This is the first Czech study on expectant management of patients with preterm premature rupture of membranes using serial transabdominal amniocentesis. To our knowledge, this is the first study prospectively evaluating Il-6 point-of-care test from serial amniocentesis in clinical management of preterm prelabour rupture of membranes. The strength of our study is also the fact that we prospectively evaluated the data of neonates born after expectant management of PPROM.

Coexistent neck enteric cyst and intraspinal neurenteric cyst: Embryopathogenetic implications Ivo Starek, Jan Herman, Richard Salzman, Petra Belakova, Daniela Skanderova

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):332-335|DOI:10.5507/bp.2020.030

The authors present an extremely rare case of coexistent enteric and neurenteric cysts of the neck. Enteric cysts alone are very sporadic in the neck region with just several cases described in the literature. Both formations are believed to be derivates of the posterior foregut. However, their embryopathogenesis has not been elucidated satisfactorily as yet. The split notochord hypothesis has been widely accepted for cysts associated with vertebral anomalies. A problem arises in cases with absent transvertebral communication between the cysts. We present a hypothesis that normal vertebrae may be formed in patients with enteric cysts.

Mechanical thrombectomy performed in thrombosed fusiform aneurysm after surgery for craniopharyngioma in adult Pavel Ryska, Miroslav Lojik, Naci Kocer, Kamil Zelenak, Tomas Cesak, Petra Cimflova, Aprajita Milind Bhorkar, Eva Vitkova, Antonin Krajina

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):336-341|DOI:10.5507/bp.2021.018

Authors present a rare case of a 45-year old women with left hemispheric stroke due to a "T" occlusion with fusiform aneurysm distal internal carotid artery after resection of the suprasellar craniopharyngioma. This vascular complication is extremely rare in adults. This case presents a combined approach of the revascularisation large vessel occlusion with that implantation of the flow diverter stent with curative effect.

Low-grade fibromyxoid sarcoma of the maxillary sinus Vladimir Koucky, David Kalfert, Daniela Kodetova Novakova, Jan Plzak

Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021, 165(3):342-345|DOI:10.5507/bp.2020.032

Low-grade fibromyxoid sarcoma is a rare tumor of the paranasal sinuses with high bleeding potential and so embolisation of the tumor prior to the surgical resection should be considered.
