Is there an alternative?In 1951, Gavriil Abramovich Ilizarov started to use the circular apparatus of his design. But the success of the brainchild of the Genius of Orthopedics was associated not only with the development of a new bone fixation system but also with a systematic approach to solving a whole complex of orthopedic problems. Its ideological core was the union of clinicians, scientists and manufacturers of metal medical products.The implementation of this concept was the creation of the world's largest medical centre with a powerful experimental base and a factory for production of the apparatus parts and medical instruments within the frame of one institution. The popularity, implementation and dissemination of the Ilizarov method were provided by a training system in which manufacturers took an active part. This model was unique for the USSR and functioned successfully only in Kurgan and Moscow (CITO), although it was and is quite common in developed countries.Foreign suppliers of implants and equipment that came to Russia de-facto organized and conduct training for orthopaedists in current techniques, retaining the market. The best Russian specialists with teaching skills also became participants in this process, thus transferring part of the foreign courses to Russia, and joined the cohort of teaching staff abroad over time. But the scientific innovative component in this chain, from the manufacturer to the surgeon, exists outside of Russia. The introduction of new technologies, described above, could hardly have other alternatives in the economic, and most importantly, existing at that time conditions of the development of such a high-tech branch of medicine in a closed USSR. But today, Russian orthopaedists have accumulated much experience and knowledge and the country needs to develop innovative sectors of the national economy.Active participation in the creation and implementation of new products and technologies should be alternative to consumption. This question is not only an economic feasibility as the preservation and development of our specialty and the security of the country as a whole. A “qualified consumer” can be easily replaced and easily managed, unlike a passionate creator.The synergy of orthopaedists and manufacturers is hindered by many factors, but the most critical are legal. The current legislative system does not ensure the safe and effective interaction of orthopaedists and manufacturers. This interaction must be deduced from the gray zone of double interpretation. The imperfection of the legislation in the sphere of tissue preservation and sterilization destroyed this once well-developed direction throughout Russia. There are practically no examples of financially successful implementation of patents in traumatology and orthopaedics.The interest of the state in the development of substitution of imported products along with conferences and congress sections on these issues is a very important step in the advancement of our specialty.
