Our intensive programs, which may include combinations of occupational, physical, & speech therapy, are customized for each child based on their needs & goals.

From determining your individualized program to registering for a session, here’s a step-by-step guide to start your child on their new journey.

STEP 1
Intake Forms
Want to determine if your child is a candidate for intensive therapy? The first step is to fill out our intake paperwork online. Make sure you have your most current OT, PT, and speech evaluation on hand so you can upload them before submitting this form. If you have insurance you would like to be reviewed, please include an uploaded picture of the front and back of your insurance card.

Email Confirmation
After you submit your paperwork, please allow up to two weeks for our therapists to review your intake form and determine if your child is a candidate for our intensive program. At this time, they will decide what type of therapy and how many hours should be included in your individualized plan. Our intensive coordinator will send you an email with our finalized therapy recommendations.

Session Registration
After we know what type of therapy and how many hours are appropriate for your child, we can move forward with scheduling. Click below to fill out the Intensive Request Form for the session of your choice. At this time, you can also request an intensive cost breakdown, which is an estimate of what we anticipate insurance will pay regarding your specific recommended therapy plan.

Billing & Insurance
After we receive your Intensive Request form, our intensive coordinator will reach out to you with a possible opening for the session you requested. Please note, filling out this form lets us know that you are interested in a specific session. It does not guarantee you a spot on our schedule. Your spot is confirmed once you place a $1,000 deposit for the tentative schedule provided by the intensive coordinator. If the session you requested is all booked up, we will add you to the waitlist for that session date and send you an email confirmation.
