Hence, it is not surprising that pain would be a frequent concern of patients with rheumatic diseases. Treatment Update in Systemic Lupus ErythematousAlberta Y. Hoi, Eric F. MorandDOI: https://doi.org/10.1016/j.rdc.2021.04.012Publication stage: In Press Corrected ProofPreviewFull-Text HTMLPDFxThe recent updates on treatment recommendations for the management of systemic lupus erythematous have provided greater clarity in the way existing anti-inflammatory and immunomodulatory drugs are used, in treating disease activity, preventing flares, and reducing irreversible organ damage and toxicity arising from the treatments themselves. Novel therapies will provide more options in the armamentarium for treating this complex disease, but ongoing studies are needed to improve understanding of the optimal treatment algorithm to maintain quality of life and improve survival for patients.Systemic Lupus Erythematosus Classification and DiagnosisMartin Aringer, Sindhu R. JohnsonDOI: https://doi.org/10.1016/j.rdc.2021.04.011Publication stage: In Press Corrected ProofPreviewFull-Text HTMLPDFxSince the European League Against Rheumatism/American College of Rheumatology 2019 classification criteria for systemic lupus erythematosus (SLE) were published, they were externally validated by groups worldwide. In particular, the new criteria worked well also in East Asian and pediatric cohorts. Antinuclear antibodies (ANA) as an entry criterion were critically discussed, but the group of ANA-negative patients is small ( 5%) worldwide. Specificity of the criteria is dependent on correct attribution only of those criteria that are not better explained by other causes. Although the classification criteria should not be used for diagnosis, many novel aspects inform diagnostic considerations.Lupus CohortsChristopher Redmond, Omer Pamuk, Sarfaraz A. HasniDOI: https://doi.org/10.1016/j.rdc.2021.04.009Publication stage: In Press Corrected ProofPreviewFull-Text HTMLPDFxLarge cohorts with diverse ethnic backgrounds and heterogenous clinical features have provided the real-life data about the safety and efficacy of various treatment regimens for systemic lupus erythematosus (SLE). There are multiple well-established regional, national, and international lupus cohorts that have made significant contributions to the understanding of SLE. Using social media for cohort-based studies can significantly increase the outreach in a short time period for studying rare diseases such as SLE. Lack of strict inclusion criteria allows study of a broad range of patients but selection bias and incomplete data are possible in long-term cohort studies.Systemic Lupus Erythematosus Outcome Measures for Systemic Lupus Erythematosus Clinical TrialsTaraneh Tofighi, Eric Morand, Zahi ToumaDOI: https://doi.org/10.1016/j.rdc.2021.04.007Publication stage: In Press Corrected ProofPreviewFull-Text HTMLPDFxThe assessment of systemic lupus erythematosus (SLE) disease activity in clinical trials has been challenging. This is related to the wide spectrum of SLE manifestations and the heterogeneity of the disease trajectory. Currently, composite outcome measures are most commonly used as a primary endpoint while organ-specific measures are often used as secondary outcomes. In this article, we review the outcome measures and endpoints used in most recent clinical trials and explore potential avenues for further development of new measures and the refinement of existing tools.Interferons in Systemic Lupus ErythematosusSirisha Sirobhushanam, Stephanie Lazar, J. Michelle KahlenbergDOI: https://doi.org/10.1016/j.rdc.2021.04.001Publication stage: In Press Corrected ProofPreviewFull-Text HTMLPDFxSkewing of type I interferon (IFN) production and responses is a hallmark of systemic lupus erythematosus (SLE). Genetic and environmental contributions to IFN production lead to aberrant innate and adaptive immune activation even before clinical development of disease. Basic and translational research in this arena continues to identify contributions of IFNs to disease pathogenesis, and several promising therapeutic options for targeting of type I IFNs and their signaling pathways are in development for treatment of SLE patients.November 2020Health Disparities in Rheumatic Diseases: Part ICandace H.
