2020-21 Physical Form Here is the most recent physical form for 2020-21 school year. If you get a new physical for this year, it must be on this form. Disregard all old physical forms. IDOE indicates that school facilities including athletics must remain shut down until June 30. No athletics until that date or further guidance from the state. We would like to remind students involved in fall extra curricular activities of this: Your spring semester grades will determine if you are eligible to participate next fall. You will have to pass at least 6 courses this semester to be eligible. Please make sure you are completing all of your e-learning and contacting your Sam Wood signs with Indiana University for track and field. Best of  luck Sam! Quoted from the Journal Gazette: Churubusco senior Sam Wood will sign with Indiana track and field with the intention of competing in the decathlon. Wood, who also played quarterback for the Eagles football team, holds the school record in both the Student-Athletes: It was just announced that all 2019-20 Physicals that are on IHSAA forms  and dated after April 1, 2019, will be valid for the 2020-2021. This is due to the difficulty scheduling Physicals this spring and summer! Email the athletic office if you are unsure if you have a physical on file. Contact Mrs.
